Inegy (Simvastatin, Ezetimibe)

Inegy acts on lipid metabolism disorders and is indicated for several hyperlipidemic conditions.

• Hypercholesterolemia: lowers elevated LDL‑cholesterol levels.
• Mixed dyslipidemia: reduces both LDL‑cholesterol and triglycerides while modestly raising HDL‑cholesterol.
• Primary hyperlipidemia: improves overall lipid profile in patients with elevated cholesterol not due to secondary causes.
• Familial hypercholesterolemia: provides additional LDL‑cholesterol reduction when statin monotherapy is insufficient.

• Simvastatin competitively inhibits HMG‑CoA reductase, the key enzyme in hepatic cholesterol synthesis, while ezetimibe blocks the NPC1L1 transporter in the intestinal brush border, decreasing dietary and biliary cholesterol absorption. The combined effect results in reduced circulating LDL‑cholesterol and improved lipid parameters.

Common Side Effects

• Headache: mild to moderate intensity.
• Myalgia: muscle aches without weakness.
• Abdominal pain: discomfort in the stomach area.
• Diarrhea: loose stools occurring intermittently.
• Nausea: feeling of queasiness.
• Elevated liver enzymes: mild increases in ALT/AST.

Serious or Rare Side Effects

• Rhabdomyolysis: severe muscle breakdown with possible kidney injury.
• Acute liver injury: marked elevation of hepatic enzymes or jaundice.
• Severe allergic reaction: rash, swelling, or anaphylaxis.
• Myopathy: muscle weakness with elevated CK levels.
• Pancreatitis: abdominal pain with elevated pancreatic enzymes.

Before initiating therapy, consider the following precautions.

• Liver function monitoring: assess baseline and periodic ALT/AST levels.
• Muscle symptom assessment: educate patients to report unexplained muscle pain or weakness.
• Renal function evaluation: adjust use in patients with severe renal impairment.
• Contraindication in active liver disease: avoid in patients with ongoing hepatic injury.
• Pregnancy and lactation: use only if potential benefit justifies potential risk.

Inegy may interact with several medicines that affect its metabolism or increase toxicity.

Major Interactions (Avoid)

• Strong CYP3A4 inhibitors (e.g., itraconazole, clarithromycin, ritonavir): may markedly increase simvastatin exposure.
• Gemfibrozil: raises risk of myopathy and rhabdomyolysis.
• Cyclosporine: enhances both simvastatin and ezetimibe concentrations.

Moderate Interactions (Monitor Closely)

• Amiodarone: may increase simvastatin levels modestly.
• Warfarin: potential for altered anticoagulant effect; monitor INR.
• Niacin: combined use can increase risk of muscle toxicity.
• Antacids containing aluminum or magnesium: may reduce ezetimibe absorption; separate dosing by at least 2 hours.
