Inegy (Simvastatin, Ezetimibe)

Inegy is used to manage lipid disorders and lower cardiovascular risk by targeting cholesterol synthesis and absorption.

• Hypercholesterolemia: reduces LDL‑cholesterol levels and helps achieve target lipid profiles.
• Mixed dyslipidemia: lowers both LDL‑cholesterol and triglycerides while modestly raising HDL‑cholesterol.
• High‑risk cardiovascular prevention: decreases the incidence of major cardiovascular events in patients with established risk factors.

• Simvastatin inhibits HMG‑CoA reductase, the key enzyme in hepatic cholesterol synthesis, leading to up‑regulation of LDL receptors and increased clearance of circulating LDL. Ezetimibe blocks the Niemann‑Pick C1‑like 1 (NPC1L1) transporter in the intestinal brush border, reducing absorption of dietary and biliary cholesterol. The combined effect results in a greater reduction of plasma LDL‑cholesterol than either agent alone.

• Muscle symptom vigilance: advise patients to report unexplained muscle pain, tenderness, or weakness promptly.
• Drug interaction awareness: avoid concomitant use of strong CYP3A4 inhibitors and other lipid‑lowering agents that increase myopathy risk.
• Pregnancy and lactation: contraindicated in pregnancy; assess risks versus benefits if breastfeeding.
• Renal impairment: dose adjustment may be required in patients with severe renal dysfunction.
• Store at ambient temperature: keep tablets in the original container, protect from moisture and direct sunlight.

Major Interactions (Avoid)

• Strong CYP3A4 inhibitors (e.g., itraconazole, clarithromycin): may increase simvastatin exposure and risk of myopathy.
• Gemfibrozil: synergistic increase in muscle toxicity.

• Cyclosporine: elevates plasma concentrations of both components.

Moderate Interactions (Monitor Closely)

• Amiodarone: may modestly raise simvastatin levels.
• Warfarin: potential alteration of INR, requiring closer monitoring.
• Digoxin: possible increase in serum digoxin concentrations.
• Antacids containing aluminum or magnesium: may reduce ezetimibe absorption.
