Inegy (Simvastatin, Ezetimibe)

Inegy is used to manage lipid disorders by targeting cholesterol synthesis and absorption pathways. It is indicated for the following conditions:

• Hypercholesterolemia: reduces low‑density lipoprotein (LDL) cholesterol to target levels.
• Mixed dyslipidemia: lowers LDL and triglycerides while modestly raising high‑density lipoprotein (HDL).
• Familial hypercholesterolemia (heterozygous): provides additional LDL‑C reduction when statin monotherapy is insufficient.

• Simvastatin inhibits HMG‑CoA reductase, the key enzyme in hepatic cholesterol biosynthesis, leading to up‑regulation of LDL receptors and increased clearance of circulating LDL particles. Ezetimibe blocks the NPC1L1 transporter in the intestinal brush border, reducing dietary and biliary cholesterol absorption. The combined effect produces additive reductions in plasma LDL‑C and total cholesterol.

Adverse reactions may occur with any lipid‑lowering therapy. Patients should be informed of typical and serious possibilities.

Common Side Effects

• Headache: mild to moderate intensity.
• Gastrointestinal discomfort: nausea, abdominal pain, or diarrhoea.
• Myalgia: muscle aches without weakness.
• Elevated liver enzymes: transient ALT/AST rise.
• Fatigue: general tiredness.
• Dizziness: occasional light‑headedness.

Serious or Rare Side Effects

• Rhabdomyolysis: severe muscle breakdown with dark urine.
• Acute liver injury: marked ALT/AST elevation with jaundice.
• Severe allergic reaction: rash, angioedema, or anaphylaxis.
• Hepatic failure: signs of liver dysfunction.
• Myopathy with weakness: muscle pain accompanied by reduced strength.
• Renal impairment: acute kidney injury secondary to rhabdomyolysis.

Before initiating therapy, clinicians should assess baseline parameters and monitor patients for safety signals.

• Liver function monitoring: obtain baseline ALT/AST and repeat periodically.
• Muscle symptom assessment: educate patients to report unexplained muscle pain or weakness.
• Contraindication in active liver disease: avoid use in patients with ongoing hepatic impairment.
• Dose adjustment in renal impairment: consider reduced dosing in severe renal dysfunction.
• Avoid concomitant strong CYP3A4 inhibitors: these may increase simvastatin exposure.
• Store at ambient temperature: keep tablets below 25 °C in the original container, protected from moisture and light.

Inegy may interact with other medicines, altering its efficacy or safety profile.

Major Interactions (Avoid)

• Strong CYP3A4 inhibitors (e.g., itraconazole, clarithromycin): markedly increase simvastatin levels.
• Gemfibrozil: raises risk of myopathy and rhabdomyolysis.
• Cyclosporine: enhances both simvastatin and ezetimibe exposure.

Moderate Interactions (Monitor Closely)

• Amlodipine: may increase simvastatin plasma concentration modestly.
• Warfarin: potential for altered anticoagulant effect; monitor INR.
• Antacids (aluminum‑magnesium): may reduce ezetimibe absorption; separate dosing by at least 2 hours.
