Inegy (Simvastatin, Ezetimibe)

Inegy combines a statin and a cholesterol absorption inhibitor to treat disorders of lipid metabolism.

• Hypercholesterolemia: reduces elevated LDL‑cholesterol levels.
• Heterozygous familial hypercholesterolemia: improves lipid profile in genetically predisposed patients.
• Primary hyperlipidemia: assists in achieving target cholesterol concentrations when diet and lifestyle are insufficient.

• Simvastatin inhibits HMG‑CoA reductase, decreasing hepatic cholesterol synthesis, while ezetimibe blocks the Niemann‑Pick C1‑like 1 transporter in the intestinal brush border, reducing dietary and biliary cholesterol absorption. The combined effect lowers circulating LDL‑cholesterol and total cholesterol levels.

Inegy may cause a range of adverse reactions, most of which are mild and reversible.

Common Side Effects

• Headache: mild to moderate pain often occurring early in therapy.
• Abdominal pain: discomfort or cramps in the stomach region.
• Diarrhoea: loose stools that may be transient.
• Muscle aches: mild myalgia without weakness.
• Nausea: feeling of sickness, sometimes with loss of appetite.

Serious or Rare Side Effects

• Rhabdomyolysis: severe muscle breakdown that can lead to kidney injury.
• Hepatic enzyme elevation: significant increase in ALT or AST indicating liver stress.
• Severe allergic reaction: rash, swelling, or anaphylaxis requiring immediate care.
• Myopathy with weakness: muscle weakness accompanied by elevated CK levels.
• Pancreatitis: inflammation of the pancreas presenting with abdominal pain and vomiting.

Before initiating therapy, several safety considerations should be reviewed.

• Liver function monitoring: obtain baseline and periodic liver enzyme tests.
• Renal impairment: assess kidney function and adjust if necessary.
• Drug interaction review: evaluate concomitant medications for potential interactions.

• Cyclosporine: can elevate statin levels, increasing toxicity.

• Warfarin: may enhance anticoagulant effect, requiring INR monitoring.
• Niacin: can increase the likelihood of muscle toxicity.
