Inegy (Simvastatin, Ezetimibe)

Inegy is used to treat lipid disorders by targeting cholesterol synthesis and absorption pathways.

• Hypercholesterolemia: lowers low‑density lipoprotein (LDL) cholesterol when diet and statin alone are inadequate.
• Mixed dyslipidemia: reduces LDL cholesterol and triglycerides while modestly increasing HDL cholesterol.
• Familial heterozygous hypercholesterolemia: provides additional LDL‑C reduction in patients already on statin therapy.

• Simvastatin inhibits HMG‑CoA reductase, the key enzyme in hepatic cholesterol synthesis, while ezetimibe blocks the NPC1L1 transporter in the intestinal brush border, reducing dietary and biliary cholesterol absorption. The combined effect produces a greater reduction in circulating LDL‑C than either agent alone.

Inegy may cause a range of adverse reactions, most of which are mild and reversible.

• Myalgia: muscle aches without weakness.
• Gastrointestinal discomfort: nausea, abdominal pain, or diarrhea.

Serious or Rare Side Effects

• Rhabdomyolysis: severe muscle breakdown with dark urine.

• Severe allergic reactions: rash, itching, swelling, or anaphylaxis.
• Myopathy with marked CK elevation.
• Interstitial lung disease: rare respiratory symptoms.

Before prescribing Inegy, clinicians should evaluate patient history and laboratory values.

• Liver function monitoring: check ALT/AST before initiation and periodically thereafter.
• Renal impairment: adjust therapy in patients with severe renal dysfunction.
• Drug interactions: review concomitant medications that affect CYP3A4 or increase myopathy risk.
• Pregnancy and lactation: avoid use unless potential benefit justifies potential risk.
• Storage: Store below 25°C: keep in the original container, protect from moisture and light.

• Cyclosporine: elevates both simvastatin and ezetimibe levels, heightening adverse effect risk.

Moderate Interactions (Monitor Closely)

• Anticoagulants (warfarin): may enhance anticoagulant effect.
• Niacin: can increase risk of hepatotoxicity.
• Antacids containing aluminum/magnesium: may reduce ezetimibe absorption.
• Digoxin: monitor serum levels due to potential increase.
