• Mupirocin exerts its antibacterial activity by selectively inhibiting bacterial isoleucyl‑tRNA synthetase, an enzyme essential for protein synthesis. This blockade prevents the incorporation of isoleucine into nascent peptide chains, halting bacterial growth and leading to bacteriostatic effects at low concentrations and bactericidal activity at higher concentrations against Gram‑positive organisms such as Staphylococcus aureus and Streptococcus pyogenes. The selective mechanism limits systemic toxicity because the target enzyme is absent in human cells.

Topical application of mupirocin may be associated with local and, rarely, systemic adverse reactions.
