Intratect (Human Normal Immunoglobulin)

Intratect, an intravenous immunoglobulin, is used to replace deficient antibodies in various immune‑mediated conditions.

• Primary immunodeficiency diseases: restores IgG levels to reduce infection risk.
• Secondary immunodeficiency (e.g., chemotherapy, HIV): provides passive immunity to help prevent opportunistic infections.
• Idiopathic thrombocytopenic purpura (ITP): raises platelet counts by modulating immune destruction.
• Kawasaki disease: reduces inflammation and coronary artery complications in acute phase.
• Multifocal motor neuropathy: improves muscle strength by neutralizing pathogenic antibodies.

• Human Normal Immunoglobulin consists of pooled IgG antibodies derived from screened donors. After intravenous infusion, the IgG distributes throughout the vascular and extravascular compartments, where it binds to bacterial, viral and toxin antigens, neutralizing them. It also interacts with Fc receptors on immune cells, modulating complement activation and cytokine release, thereby down‑regulating auto‑reactive immune responses and providing passive immunity.

Intratect may be associated with a range of adverse reactions, most of which are infusion‑related or immune‑mediated.

Common Side Effects

• Headache: mild to moderate pain often occurring during or shortly after infusion.
• Fever or chills: transient rise in body temperature or feeling of cold.
• Fatigue: generalized tiredness following treatment.
• Nausea: mild gastrointestinal upset.
• Myalgia: muscle aches or soreness.
• Injection site reactions: erythema, swelling or discomfort at the infusion site.

Serious or Rare Side Effects

• Anaphylaxis: severe allergic reaction requiring immediate medical intervention.
• Thromboembolic events: blood clots that may affect veins or arteries.
• Renal impairment: acute kidney injury manifested by reduced urine output.
• Hemolytic anemia: destruction of red blood cells leading to anemia.
• Aseptic meningitis: inflammation of meninges without infection.
• Transfusion‑related acute lung injury (TRALI): acute respiratory distress following infusion.

Before administering Intratect, clinicians should consider several precautionary measures to ensure patient safety.

• Assess patient history: review prior reactions to immunoglobulin products and known IgA deficiency.
• Monitor vital signs: check blood pressure, heart rate and temperature before, during and after infusion.
• Adjust infusion rate: start slowly and increase gradually to reduce infusion‑related reactions.
• Pre‑medicate when needed: consider antihistamines or antipyretics for patients with previous mild reactions.
• Evaluate renal function: check serum creatinine in patients with pre‑existing kidney disease.
• Store at 2-8°C: keep refrigerated, protect from light, do not freeze or shake the solution.

Major Interactions (Avoid)

• Live vaccines: concurrent use may diminish vaccine efficacy or increase risk of infection.
• High‑dose aspirin or NSAIDs: may increase risk of bleeding when combined with immunoglobulin therapy.

• Corticosteroids: may potentiate immunosuppressive effects, requiring dose adjustment.
• Anticoagulants (e.g., warfarin): immunoglobulin can affect coagulation parameters, necessitating INR monitoring.
• Diuretics: may exacerbate renal stress when combined with high‑volume infusions.
