Invanz (Ertapenem)

Invanz (ertapenem) is used to treat infections caused by susceptible bacteria in several clinical categories.

• Complicated intra‑abdominal infections: achieves bacterial eradication in severe intra‑abdominal infections following surgery or perforation.
• Complicated skin and soft tissue infections: provides effective therapy for deep tissue infections such as cellulitis or necrotizing fasciitis.
• Community‑acquired pneumonia: delivers bactericidal activity against common respiratory pathogens in adult patients.
• Complicated urinary tract infections: resolves infections of the kidney and bladder caused by resistant organisms.
• Diabetic foot infection: helps clear polymicrobial infections in diabetic foot ulcers.

• Ertapenem exerts its antibacterial effect by binding to penicillin‑binding proteins (PBPs) located on the bacterial cell wall. This binding inhibits the final transpeptidation step of peptidoglycan synthesis, weakening the cell wall and leading to osmotic lysis. The drug displays high affinity for a broad range of PBPs in both Gram‑negative and Gram‑positive bacteria, resulting in rapid bactericidal activity and is stable against most β‑lactamases.

Ertapenem may be associated with a range of adverse reactions, varying from mild to severe.

Serious or Rare Side Effects

• Seizures, especially in patients with CNS disorders

• Renal function assessment: evaluate creatinine clearance before initiating therapy and adjust dose in renal impairment.
• Hypersensitivity monitoring: watch for signs of allergic reaction such as rash, pruritus or anaphylaxis.
• Seizure risk: avoid use in patients with a history of seizures or CNS disorders unless benefits outweigh risks.
• Valproic acid interaction: discontinue or replace valproic acid, as ertapenem can reduce its plasma concentrations.
• Concomitant nephrotoxic agents: monitor renal parameters when used with other nephrotoxic drugs.
• Store at 2-8°C: keep refrigerated, protect from light, do not freeze or shake.
