Inzolfi (Fingolimod)

Inzolfi is used in the management of multiple sclerosis, a chronic autoimmune disease that leads to demyelination and neurological disability. Clinical trials have demonstrated its efficacy in reducing disease activity.

• Relapsing‑remitting multiple sclerosis: reduces the frequency of clinical relapses and slows the accumulation of disability.

• After oral absorption, fingolimod is phosphorylated to fingolimod‑phosphate, which binds with high affinity to sphingosine‑1‑phosphate receptors 1, 3, 4 and 5 on lymphocytes. This binding functionally antagonizes the receptors, preventing lymphocyte egress from lymph nodes, thereby decreasing the number of circulating autoreactive lymphocytes that can infiltrate the central nervous system and cause inflammation. The resultant reduction in CNS immune surveillance translates into lower relapse rates and slower disease progression.

Common Side Effects

• Headache: mild to moderate headache.
• Diarrhea: loose stools.
• Upper respiratory tract infection: nasopharyngitis.
• Elevated liver enzymes: transient increase in ALT/AST.

Serious or Rare Side Effects

• Severe infections: opportunistic infections such as herpes zoster.
• Cardiac effects: bradyarrhythmia or atrioventricular block, especially after first dose.
• Macular edema: vision changes due to retinal swelling.

• Baseline cardiac assessment: perform ECG and monitor heart rate before initiating therapy.
• First‑dose monitoring: observe patients for at least six hours for bradycardia or conduction abnormalities.
• Infection screening: evaluate for active infections and vaccinate appropriately before treatment.
• Liver function testing: check ALT/AST levels regularly during therapy.
• Pregnancy considerations: avoid use in pregnant women; discuss contraception.
