Ipinzan (Metformin Hydrochloride, Vildagliptin)

Ipinzan is an oral antidiabetic medication used to improve glycaemic control in type 2 diabetes mellitus.

• Type 2 diabetes mellitus: lowers fasting and post‑prandial glucose when combined with diet and exercise.
• Inadequate response to metformin alone: provides additional glucose‑lowering effect through DPP‑4 inhibition.
• Patients requiring combination therapy: reduces HbA1c by targeting hepatic glucose production and incretin pathways.

• Metformin reduces hepatic gluconeogenesis and enhances peripheral insulin sensitivity, while Vildagliptin inhibits the enzyme dipeptidyl peptidase‑4 (DPP‑4). This inhibition raises endogenous incretin levels, leading to glucose‑dependent insulin secretion and suppressed glucagon release, together achieving better glycaemic control.

Common Side Effects

• Nausea: mild to moderate gastrointestinal upset.

• Headache: mild tension‑type headache.
• Hypoglycaemia (when combined with sulfonylureas or insulin): symptomatic low blood sugar.

Serious or Rare Side Effects

• Lactic acidosis: life‑threatening metabolic acidosis, especially in renal impairment.
• Severe hypoglycaemia: prolonged low blood glucose requiring medical intervention.
• Pancreatitis: acute inflammation of the pancreas.
• Hepatic dysfunction: elevated liver enzymes or hepatitis.
• Allergic reactions: rash, pruritus, or angio‑edema.
• Stevens‑Johnson syndrome: severe skin reaction.

Before initiating Ipinzan, clinicians should consider several safety measures.

• Renal function monitoring: assess creatinine clearance to avoid metformin accumulation.
• Hepatic function assessment: evaluate liver enzymes due to risk of lactic acidosis.
• Elderly patients: start at lower dose and monitor for adverse effects.
• Metabolic acidosis risk: avoid in patients with conditions predisposing to acidosis.
• Concomitant hypoglycaemic agents: adjust doses to prevent severe hypoglycaemia.
