Ipraalox (Pantoprazole)

Pantoprazole is a proton pump inhibitor that reduces gastric acid secretion in several acid‑related conditions.

• Gastro‑oesophageal reflux disease: provides symptomatic relief and promotes healing of the oesophageal lining.
• Erosive esophagitis: accelerates mucosal healing and reduces further erosion.
• Peptic ulcer disease: supports ulcer healing and prevents recurrence.
• Helicobacter pylori eradication regimens: enhances the effectiveness of antibiotic‑based therapy.

• Pantoprazole selectively inhibits the H+/K+‑ATPase enzyme (the gastric proton pump) located on the secretory surface of parietal cells. By blocking this final step of acid production, it markedly reduces both basal and stimulated gastric acid output, allowing the gastrointestinal mucosa to heal.

Pantoprazole is generally well tolerated, but adverse reactions can occur.

Common Side Effects

• Headache: mild, transient headache.
• Diarrhoea: loose stools without blood.
• Nausea: mild stomach upset.
• Abdominal pain: cramping or discomfort.
• Flatulence: increased gas.
• Constipation: reduced bowel movements.

Serious or Rare Side Effects

• Severe allergic reaction: rash, itching, swelling, or difficulty breathing.
• Clostridioides difficile infection: persistent watery diarrhoea.
• Low magnesium levels: muscle cramps, irregular heartbeat.
• Liver enzyme elevation: asymptomatic rise in ALT/AST.
• Fracture risk: decreased bone mineral density with long‑term use.

When using Pantoprazole, consider the following precautions:

• Assess renal function: adjust use if severe impairment is present.
• Monitor for infections: be alert for signs of C. difficile infection.
• Evaluate magnesium status: periodic testing is advised for long‑term therapy.
• Review bone health: consider calcium/vitamin D supplementation if therapy exceeds 12 months.
• Store at below 25°C: keep in the original packaging, protect from moisture and direct sunlight.

Pantaprazole may interact with several medicines, requiring careful management.

Major Interactions (Avoid)

• Antifungal azoles (e.g., ketoconazole): reduced absorption of the azole.
• HIV protease inhibitors: decreased efficacy of the antiviral.
• Clopidogrel: diminished antiplatelet effect.

Moderate Interactions (Monitor Closely)

• Warfarin: potential alteration of INR values.
• Methotrexate: increased serum levels.
• Digoxin: possible rise in serum digoxin concentrations.
• Phenytoin: altered metabolism of the anti‑epileptic.
