Iscover (Clopidogrel)

Clopidogrel is an antiplatelet agent used to reduce thrombotic events in several cardiovascular conditions.

• Myocardial infarction: lowers the risk of recurrent heart attacks in patients with prior MI.
• Ischemic stroke: diminishes the chance of a subsequent stroke in individuals who have experienced an ischemic event.
• Peripheral arterial disease: helps prevent limb‑related ischemic complications and cardiovascular events in patients with PAD.

• Clopidogrel is an irreversible antagonist of the platelet P2Y12 ADP receptor. By binding covalently to this receptor, it blocks ADP‑mediated activation of the glycoprotein IIb/IIIa complex, thereby inhibiting platelet aggregation and reducing thrombus formation in arterial circulation.

Clopidogrel may cause a range of adverse reactions, from mild to serious.

Common Side Effects

• Bleeding gums or easy bruising
• Nosebleeds (epistaxis)
• Diarrhea or abdominal discomfort
• Skin rash or itching

Serious or Rare Side Effects

• Severe or uncontrolled bleeding (e.g., gastrointestinal, intracranial)
• Thrombotic thrombocytopenic purpura (TTP)
• Anaphylactic or severe allergic reactions
• Significant liver enzyme elevations
• Neutropenia or other blood dyscrasias

Before initiating clopidogrel therapy, clinicians should evaluate individual risk factors and monitor patients regularly.

• Assess bleeding risk: review history of gastrointestinal ulcers or recent surgery.
• Monitor liver function: perform periodic hepatic enzyme tests.
• Adjust in renal impairment: consider dose modification in severe renal dysfunction.
• Avoid in active pathological bleeding: discontinue if major bleeding occurs.
• Store at room temperature: keep in the original pack, protect from moisture and light.

Clopidogrel interacts with several drug classes that can alter its antiplatelet effect or increase bleeding risk.

Major Interactions (Avoid)

• CYP2C19 inhibitors (e.g., omeprazole, esomeprazole) may reduce activation of clopidogrel.
• Concurrent strong antiplatelet agents (e.g., aspirin, ticagrelor) increase bleeding risk.
• Non‑steroidal anti‑inflammatory drugs (NSAIDs) such as ibuprofen can exacerbate bleeding.
• Warfarin or other anticoagulants may lead to excessive hemorrhage.
• Selective serotonin reuptake inhibitors (SSRIs) may potentiate bleeding.

Moderate Interactions (Monitor Closely)

• CYP3A4 inducers (e.g., rifampin, carbamazepine) may lower clopidogrel effectiveness.
• Certain proton pump inhibitors (e.g., pantoprazole) have modest impact on platelet inhibition.
• Statins (e.g., atorvastatin) can increase plasma levels of clopidogrel's active metabolite.
