Isodiur (Torasemide)

Isodiur, a loop diuretic, is employed in conditions characterized by fluid retention.

• Heart failure‑related edema: reduces excess fluid accumulation by promoting diuresis.
• Liver cirrhosis‑associated edema: helps remove ascitic fluid and peripheral swelling.
• Renal disease‑related edema: facilitates removal of retained water and salts.
• Hypertension: lowers blood pressure by decreasing plasma volume.

• Torasemide inhibits the Na+/K+/2Cl‑ cotransporter in the thick ascending limb of the loop of Henle. This blockade prevents reabsorption of sodium, potassium, and chloride, leading to increased excretion of these electrolytes together with water, thereby reducing extracellular fluid volume and blood pressure.

Common Side Effects

• Dizziness: sensation of light‑headedness, especially after standing.
• Headache: mild to moderate pain in the head region.
• Increased urination: frequent need to void.
• Electrolyte imbalance: low potassium or sodium levels.
• Nausea: feeling of stomach upset.
• Dehydration: excessive fluid loss leading to dry mouth or thirst.

Serious or Rare Side Effects

• Severe dehydration: marked fluid deficit requiring medical attention.
• Acute renal failure: sudden loss of kidney function.
• Ototoxicity: hearing impairment or ringing in the ears.
• Severe electrolyte disturbances: dangerous drops in potassium or sodium.
• Hypersensitivity reactions: rash, itching, or swelling.
• Blood dyscrasias: abnormal blood cell counts.

Before initiating therapy, consider the following precautions.

• Monitor electrolytes: check serum potassium and sodium regularly.
• Assess renal function: evaluate creatinine clearance prior to use.
• Use cautiously in hepatic impairment: dose adjustment may be required.
• Avoid in patients with anuria: drug ineffective and may cause harm.
• Pregnancy and lactation: safety not established; use only if benefit outweighs risk.
• Store at room temperature: keep in the original pack, protect from moisture and light.
