Isopto-Carpine acts on the ocular muscarinic pathway to lower intra‑ocular pressure and facilitate diagnostic procedures.

• Open‑angle glaucoma: reduces intra‑ocular pressure by enhancing aqueous outflow.
• Ocular hypertension: lowers eye pressure to prevent progression to glaucoma.
• Diagnostic miosis: produces pupil constriction to improve visualization during ocular examinations.

• Pilocarpine hydrochloride is a direct agonist of muscarinic receptors in the eye. By stimulating the sphincter pupillae muscle, it causes pupil constriction and contracts the ciliary muscle, which opens the trabecular meshwork and increases aqueous humor outflow, thereby decreasing intra‑ocular pressure.

Adverse reactions may occur with ocular use of pilocarpine.

Common Side Effects

• Blurred vision: temporary difficulty focusing after instillation.
• Eye irritation or burning: mild discomfort at the site of application.
• Increased lacrimation: excess tearing.
• Headache: mild to moderate pain often related to ciliary spasm.
• Photophobia: sensitivity to light due to pupil constriction.

Serious or Rare Side Effects

• Severe ocular inflammation: uveitis or iritis requiring immediate care.

• Allergic reaction: angio‑edema, rash or conjunctival swelling.
• Systemic cholinergic toxicity: bradycardia, hypotension or bronchospasm in susceptible individuals.
