Isotretinoin is a retinoid used in the management of severe acne conditions.

• Severe nodular acne: reduces lesion count and promotes clearance of nodules.
• Acne vulgaris refractory to other therapies: decreases sebum production and inflammation, leading to clinical improvement.
• Acne conglobata: helps achieve reduction of extensive inflammatory lesions.

• Isotretinoin binds to nuclear retinoic acid receptors (RARs) and retinoid X receptors (RXRs), altering gene transcription. This modulation reduces sebaceous gland size, lowers sebum output, normalizes follicular keratinization, and exerts anti‑inflammatory effects, collectively addressing the pathogenic factors of severe acne.

Common Side Effects

• Dry skin and mucous membranes: dryness of skin, lips (cheilitis), and eyes.
• Elevated liver enzymes: mild transaminase increases detectable on routine labs.
• Hypertriglyceridemia: rise in blood triglyceride levels.
• Photosensitivity: increased susceptibility to sunburn.
• Musculoskeletal discomfort: joint or muscle aches.
• Headache: mild to moderate intensity.

Serious or Rare Side Effects

• Teratogenicity: severe birth defects if exposure occurs during pregnancy.
• Depression or suicidal ideation: mood changes requiring immediate evaluation.
• Inflammatory bowel disease: new onset or exacerbation of ulcerative colitis or Crohn's disease.
• Severe liver injury: marked elevation of hepatic enzymes or hepatitis.
• Pancreatitis: acute inflammation of the pancreas.
• Pseudotumor cerebri: increased intracranial pressure presenting with headache and visual changes.

• Pregnancy contraindication: drug is category X and must not be used during pregnancy.
• Teratogenic risk: ensure effective contraception before, during, and for at least one month after discontinuation.
