Isotretinoin Orifarm (Isotretinoin)

Isotretinoin Orifarm is a retinoid indicated for severe forms of acne that are resistant to conventional therapy.

• Severe recalcitrant nodular acne: reduces inflammatory lesions and prevents formation of new nodules.
• Severe acne vulgaris: decreases comedones, papules and pustules when other treatments have failed.

• Isotretinoin binds to nuclear retinoic acid receptors, modulating gene expression that leads to reduced sebaceous gland size and sebum output, normalization of follicular keratinization, and anti‑inflammatory effects, thereby addressing the pathogenic factors of severe acne.

Isotretinoin may cause a range of adverse reactions, from mild to serious.

Common Side Effects

• Dry skin: increased epidermal dryness and scaling.
• Cheilitis: cracking and inflammation of the lips.
• Xerosis of mucous membranes: dryness of eyes and nasal passages.
• Photosensitivity: heightened skin reaction to sunlight.
• Headache: mild to moderate cephalic discomfort.
• Nasal bleeding: occasional epistaxis due to mucosal dryness.

Serious or Rare Side Effects

• Severe depression or suicidal ideation: mood alterations requiring immediate medical attention.
• Liver enzyme elevation: hepatotoxicity detectable by laboratory monitoring.
• Hypertriglyceridemia: significant rise in serum triglycerides.
• Teratogenicity: risk of birth defects if exposure occurs during pregnancy.
• Pseudotumor cerebri: increased intracranial pressure presenting with headache and visual changes.
• Severe skin reactions: rare cases of Stevens‑Johnson syndrome or toxic epidermal necrolysis.

Before initiating therapy, several safety measures should be observed.

• Pregnancy testing: confirm a negative test and maintain effective contraception throughout treatment.
• Baseline labs: obtain liver function tests and lipid profile prior to start and monitor periodically.
• Psychiatric monitoring: assess for depression, anxiety, or mood changes during therapy.
• Avoid vitamin A supplementation: concurrent use may increase risk of toxicity.
• Store at ambient temperature: keep below 25°C in the original packaging, protect from moisture.

Isotretinoin can interact with several medicines, requiring attention to avoid reduced efficacy or increased toxicity.

Major Interactions (Avoid)

• Tetracyclines: concurrent use raises risk of intracranial hypertension.
• Vitamin A derivatives: additive teratogenic risk.

Moderate Interactions (Monitor Closely)

• Oral contraceptives: may reduce contraceptive effectiveness; consider alternative methods.
