Izgrev (Hydrochlorothiazide, Irbesartan)

Izgrev, a combination of a thiazide diuretic and an angiotensin II receptor blocker, is indicated for conditions related to high blood pressure.

• Primary hypertension: lowers systolic and diastolic blood pressure to reduce cardiovascular risk.
• Hypertension requiring diuretic therapy: adds diuretic effect to improve fluid balance and blood pressure control.
• Hypertension with additional cardiovascular risk: provides dual mechanism to achieve better long‑term blood pressure management.

• Hydrochlorothiazide inhibits sodium‑chloride reabsorption in the distal convoluted tubule, increasing urinary excretion of water and electrolytes, which reduces plasma volume. Irbesartan selectively blocks the AT1 receptor, preventing angiotensin II‑mediated vasoconstriction and aldosterone release, leading to vasodilation and further reduction of blood pressure.

Adverse reactions may occur with any antihypertensive therapy and should be monitored.

Common Side Effects

• Dizziness: transient light‑headedness, especially after the first dose.
• Increased urination: due to diuretic action, may cause nocturia.
• Electrolyte imbalance: mild changes in potassium or sodium levels.
• Headache: occasional mild to moderate intensity.
• Fatigue: general tiredness during treatment initiation.

Serious or Rare Side Effects

• Severe dehydration: excessive fluid loss leading to hypotension.
• Hyperkalemia: elevated potassium that can cause cardiac arrhythmias.
• Acute renal impairment: sudden decrease in kidney function.
• Angioedema: swelling of face, lips, or throat requiring immediate medical attention.
• Severe hypotension: marked drop in blood pressure causing fainting or shock.

Before initiating therapy, consider the following safety measures.

• Electrolyte monitoring: check serum potassium and sodium regularly.
• Renal function assessment: evaluate creatinine and eGFR before and during treatment.
• Elderly patients: start at lower doses and monitor for orthostatic hypotension.
• Pregnancy and lactation: safety not established; use only if clearly needed.
• Concomitant medications: review for drugs that affect potassium or renal function.
• Store at room temperature: keep in the original container, protect from moisture and direct sunlight.

Major Interactions (Avoid)

• Lithium: increased lithium levels may cause toxicity.
• ACE inhibitors or other ARBs: combined use can lead to severe hyperkalemia and renal dysfunction.
