Jext (Epinephrine Bitartrate)

Jext, containing epinephrine, is used in the emergency management of conditions involving acute allergic or bronchospastic responses.

• Anaphylaxis: rapid reversal of airway obstruction, hypotension, and swelling.
• Severe allergic reactions: immediate mitigation of systemic symptoms and prevention of progression to anaphylaxis.
• Acute bronchospasm: prompt bronchodilation to improve airflow in severe asthma or other bronchospastic events.

• Epinephrine acts as a non‑selective agonist at α1, α2, β1 and β2 adrenergic receptors. Stimulation of α1 receptors induces vasoconstriction, raising systemic vascular resistance and blood pressure. β1 activation increases cardiac contractility and heart rate, while β2 stimulation relaxes bronchial smooth muscle and inhibits mediator release from mast cells, collectively counteracting the pathophysiology of anaphylaxis and bronchospasm.

Epinephrine may cause a range of adverse reactions, varying from mild to life‑threatening.

Common Side Effects

• Tremor: involuntary shaking, especially of the hands.
• Anxiety or nervousness: heightened sense of alertness or panic.
• Headache: mild to moderate pain in the head.
• Nausea or vomiting: gastrointestinal discomfort following injection.

Serious or Rare Side Effects

• Severe hypertension: sudden, dangerous rise in blood pressure.
• Cardiac arrhythmias: irregular heart rhythms that may be life‑threatening.
• Myocardial ischemia or infarction: reduced blood flow to the heart muscle.
• Pulmonary edema: fluid accumulation in the lungs causing respiratory distress.

Before using Jext, consider the following precautions to ensure safe and effective administration.

• Assess cardiovascular status: avoid in patients with uncontrolled hypertension, recent myocardial infarction, or serious cardiac disease.
• Check for hypersensitivity: do not use if the patient has known allergy to epinephrine or any excipients in the autoinjector.
• Use only for indicated emergencies: it is not a substitute for routine bronchodilator therapy in chronic asthma.
• Avoid concurrent β‑blockers: these may diminish therapeutic effect and increase risk of severe hypertension.
• Pregnancy and lactation: administer only if the potential benefit justifies the potential risk to the fetus or infant.
• Store below 25°C: keep in the original packaging, protect from moisture and light, and do not freeze.

Epinephrine can interact with several medicines, altering its efficacy or safety profile.

Major Interactions (Avoid)

• β‑blockers: may blunt epinephrine’s bronchodilator effect and precipitate severe hypertension.
• Monoamine oxidase inhibitors (MAOIs): can potentiate sympathomimetic actions, increasing risk of cardiovascular events.
• Tricyclic antidepressants: may enhance epinephrine‑induced tachycardia and hypertension.
• Ergot alkaloids: combined use can cause severe vasospasm.

Moderate Interactions (Monitor Closely)

• Diuretics: may predispose to hypokalemia, heightening arrhythmia risk when epinephrine is administered.
• Theophylline: additive bronchodilator effect may lead to tachyarrhythmias; monitor cardiac status.
• Other sympathomimetics (e.g., pseudoephedrine): combined stimulation can increase cardiovascular strain.
