Jimandin (Sitagliptin)

Sitagliptin is a DPP‑4 inhibitor used in the management of type 2 diabetes mellitus, acting on the incretin pathway to lower blood glucose.

• Type 2 diabetes mellitus: improves glycemic control as monotherapy when diet and exercise are insufficient.
• Combination therapy with metformin: provides additional HbA1c reduction in patients inadequately controlled on metformin alone.
• Combination with insulin or sulfonylureas: helps achieve target glucose levels while allowing lower doses of insulin or sulfonylureas.

• Sitagliptin selectively inhibits the dipeptidyl peptidase‑4 (DPP‑4) enzyme, preventing the breakdown of endogenous incretin hormones such as GLP‑1 and GIP. Elevated incretin levels enhance glucose‑dependent insulin secretion, suppress glucagon release, and slow gastric emptying, resulting in reduced post‑prandial and fasting blood glucose concentrations.

Common Side Effects

• Nasopharyngitis: mild upper respiratory tract infection symptoms.
• Headache: transient mild to moderate headache.
• Upper respiratory tract infection: cough, sore throat, or congestion.
• Nausea: occasional mild nausea.
• Diarrhea: loose stools, usually self‑limiting.
• Arthralgia: mild joint pain.

Serious or Rare Side Effects

• Pancreatitis: severe abdominal pain requiring immediate medical attention.
• Severe allergic reactions: rash, angioedema, or anaphylaxis.
• Bullous pemphigoid: blistering skin disorder.
• Joint swelling or pain: may indicate autoimmune reaction.
• Hypoglycemia: especially when combined with insulin or sulfonylureas.

Before initiating therapy, consider the following precautions:

• Renal function: adjust dose in patients with severe renal impairment.
• Hepatic impairment: use with caution in moderate to severe liver disease.
• History of pancreatitis: avoid use and monitor for symptoms.
• Concomitant insulin or sulfonylureas: monitor blood glucose to prevent hypoglycemia.
• Pregnancy and breastfeeding: limited data; use only if potential benefit justifies the risk.
• Store at ambient temperature: keep in the original container, protect from moisture and direct sunlight.
