Jimandin (Sitagliptin)

• Jimandin belongs to the DPP‑4 inhibitor class and is indicated for managing hyperglycaemia in type 2 diabetes.
• Type 2 diabetes mellitus: improves glycaemic control when combined with diet and exercise.
• Adjunct therapy with metformin: provides additional HbA1c reduction in patients inadequately controlled on metformin alone.

• Sitagliptin selectively inhibits the enzyme dipeptidyl peptidase‑4 (DPP‑4), which degrades the incretin hormones GLP‑1 and GIP. By blocking DPP‑4, sitagliptin increases endogenous incretin levels, enhancing glucose‑dependent insulin secretion and suppressing glucagon release, thereby lowering post‑prandial and fasting plasma glucose concentrations.

Common Side Effects

• Nasopharyngitis: mild inflammation of the nasal passages and throat.
• Headache: transient cranial pain of varying intensity.
• Upper respiratory tract infection: symptoms such as cough and sore throat.
• Nausea: occasional feeling of stomach upset.
• Diarrhoea: loose stools occurring intermittently.
• Abdominal pain: mild discomfort in the stomach area.

Serious or Rare Side Effects

• Pancreatitis: severe abdominal pain with possible vomiting, requiring immediate medical attention.
• Severe allergic reactions: rash, angioedema, or anaphylaxis.
• Joint pain: unexplained arthralgia that may be persistent.
• Hepatic injury: elevated liver enzymes indicating possible liver stress.
• Hypoglycaemia (when combined with insulin or sulfonylureas): low blood sugar episodes.
• Bullous pemphigoid: rare skin blistering disorder.

• Patients should be evaluated for renal function before initiating therapy and monitored periodically.
• Renal function assessment: adjust dose or avoid use in severe impairment.
• Monitor for pancreatitis: discontinue if severe abdominal pain occurs.
• Caution in heart failure: assess fluid status regularly.
• Avoid in known hypersensitivity: discontinue if allergic reaction develops.
• Use with caution in hepatic impairment: monitor liver enzymes.
