Jivolar (Metformin Hydrochloride, Sitagliptin)

Jivolar combines metformin and sitagliptin to target hyperglycaemia in type 2 diabetes mellitus.

• Type 2 diabetes mellitus (monotherapy): achieves improved fasting and post‑prandial glucose levels.
• Type 2 diabetes mellitus (add‑on to metformin): provides additional HbA1c reduction when metformin alone is insufficient.
• Type 2 diabetes mellitus (with diet and exercise): supports overall glycaemic control as part of comprehensive management.

• Metformin reduces hepatic gluconeogenesis and enhances peripheral insulin sensitivity, thereby decreasing basal glucose output. Sitagliptin selectively inhibits the dipeptidyl peptidase‑4 (DPP‑4) enzyme, prolonging the activity of incretin hormones such as GLP‑1 and GIP, which augment glucose‑dependent insulin secretion and suppress glucagon release. The complementary actions of both agents provide additive glucose‑lowering effects without causing hypoglycaemia when used alone.

Common Side Effects

• Nausea: mild gastrointestinal discomfort.
• Diarrhoea: loose stools, often transient.
• Headache: transient cranial pain.

Serious or Rare Side Effects

• Lactic acidosis: rare but potentially fatal metabolic disturbance.
• Severe hypoglycaemia: especially when combined with insulin or sulfonylureas.
• Pancreatitis: acute pancreatic inflammation with abdominal pain.

When prescribing Jivolar, several safety considerations should be observed.

• Renal function assessment: evaluate creatinine clearance before initiating and periodically during therapy.
• Hepatic impairment monitoring: use with caution in patients with liver disease due to lactic acidosis risk.
• Risk of lactic acidosis: avoid in patients with conditions predisposing to hypoxia or severe infection.
• Cardiovascular disease: monitor closely in patients with heart failure.
• Elderly patients: start at lower dose and titrate cautiously.
• Store at room temperature: keep in original container, protect from moisture and direct sunlight.

Jivolar can interact with several medicines, requiring attention to avoid adverse outcomes.

Major Interactions (Avoid)

• Iodinated contrast agents: may precipitate renal dysfunction and increase metformin accumulation.
• Cimetidine: can raise metformin plasma concentrations.
