Karbidopa/Levodopa Medochemie (Carbidopa, Levodopa)

Karbidopa/Levodopa is used within the Parkinsonian disease pathway to restore dopaminergic activity and improve motor function.

• Parkinson's disease: reduces bradykinesia, rigidity and tremor, improving daily activities.
• Parkinsonian syndrome: alleviates motor impairment caused by secondary or atypical parkinsonism.
• Levodopa‑responsive Parkinsonism: provides symptomatic control when dopamine deficiency is the primary driver.

• Levodopa crosses the blood‑brain barrier and is decarboxylated to dopamine within the central nervous system, replenishing depleted neurotransmitter stores. Carbidopa inhibits peripheral aromatic L‑amino acid decarboxylase, preventing conversion of levodopa outside the brain, which increases central availability and reduces peripheral side effects such as nausea and hypotension.

Adverse reactions may occur with any antiparkinson therapy and should be monitored during treatment.

Common Side Effects

• Nausea: mild to moderate gastrointestinal upset.

• Dose titration: start low and increase gradually to reduce dyskinesia.
• Renal or hepatic impairment: adjust dose and monitor for accumulation.
• Narrow‑angle glaucoma: avoid use as it may increase intra‑ocular pressure.
• Psychiatric history: screen for hallucinations or mood changes before initiation.
• Concomitant antipsychotics: avoid dopamine‑blocking agents that diminish efficacy.
• Store at ambient temperature: keep in original container, protect from moisture and direct sunlight.

Major Interactions (Avoid)

• Non‑selective MAO inhibitors (e.g., phenelzine): risk of hypertensive crisis.
