Indications & Clinical Uses

Kardogrel (clopidogrel hydrogen sulfate) is used to reduce thrombotic risk in several cardiovascular and cerebrovascular conditions.

  • Acute coronary syndrome: helps prevent atherothrombotic events such as myocardial infarction.
  • Ischemic stroke or transient ischemic attack history: lowers the chance of recurrent stroke.
  • Peripheral arterial disease: reduces incidence of myocardial infarction and other arterial events.
  • Post‑myocardial infarction: provides secondary prevention of further cardiovascular events.

How It Works

  • Clopidogrel hydrogen sulfate is a pro‑drug that is metabolized in the liver to an active thiol metabolite. The metabolite irreversibly binds to the platelet P2Y12 ADP receptor, blocking ADP‑mediated activation of the GPIIb/IIIa complex and thus preventing platelet aggregation and thrombus formation. By inhibiting this final common pathway of platelet activation, clopidogrel reduces the likelihood of arterial clot formation without significantly affecting coagulation factors.

Side Effects

Clopidogrel may be associated with a range of adverse reactions, varying from mild to serious.

Common Side Effects

  • Bleeding gums: mild oral bleeding that may increase with brushing.
  • Bruising: easy bruising or petechiae, especially after minor trauma.
  • Nausea: occasional nausea, sometimes accompanied by mild stomach discomfort.

Serious or Rare Side Effects

  • Severe bleeding: gastrointestinal or intracranial hemorrhage that may be life‑threatening.
  • Thrombocytopenia: a marked reduction in platelet count, increasing bleeding risk.
  • Allergic reactions: rash, pruritus, or anaphylaxis requiring immediate medical attention.
  • Liver enzyme elevation: increased ALT/AST indicating possible hepatic irritation.
  • Neutropenia: reduced neutrophil count that may predispose to infection.

Precautions & Warnings

When prescribing Kardogrel, clinicians should consider several precautionary measures to ensure safe use.

  • Assess bleeding risk: evaluate patient history of hemorrhage before initiating therapy.
  • Monitor platelet count: regular checks are advised during treatment.
  • Adjust in renal impairment: dose may need modification in severe kidney disease.
  • Caution in hepatic disease: use carefully in patients with liver dysfunction.
  • Avoid in hypersensitivity: discontinue if allergic reaction occurs.
  • Store at ambient temperature: keep below 25 °C in original packaging, protect from moisture.

Avoid Interactions

Clopidogrel interacts with several medicines; awareness of these interactions helps prevent reduced efficacy or increased toxicity.

Major Interactions (Avoid)

  • Proton pump inhibitors (omeprazole, esomeprazole): may diminish clopidogrel activation.
  • Strong CYP2C19 inhibitors (fluoxetine, fluconazole): reduce conversion to active metabolite.
  • Anticoagulants (warfarin, direct oral anticoagulants): increase bleeding risk.

Moderate Interactions (Monitor Closely)

  • NSAIDs (ibuprofen, naproxen): additive bleeding potential.
  • SSRIs/SNRIs (sertraline, venlafaxine): may increase hemorrhagic risk.
  • Statins (atorvastatin, simvastatin): potential for enhanced antiplatelet effect.
