Ketrel (Tretinoin)

Tretinoin is a topical retinoid that modulates skin cell growth and inflammation, making it useful for several dermatological conditions.

• Acne vulgaris: Reduces comedones and inflammatory lesions by normalizing follicular keratinization.
• Photodamage (skin aging): Improves fine lines, hyperpigmentation and texture by stimulating collagen synthesis and epidermal turnover.
• Hyperkeratinizing disorders (e.g., psoriasis, ichthyosis): Softens plaques and reduces scaling through regulation of keratinocyte differentiation.

• Tretinoin binds to nuclear retinoic acid receptors (RARs) and retinoid X receptors (RXRs) in skin cells, altering transcription of genes that control keratinocyte proliferation, differentiation and inflammatory mediators. This results in increased epidermal turnover, reduced hyperkeratinization, and modulation of cytokine production, which together improve acne, photodamaged skin and hyperkeratinizing lesions.

Topical tretinoin can cause a range of skin reactions, most of which are mild and reversible, while rare events may require medical attention.

Common Side Effects

• Irritation: Redness, burning or stinging at the application site.
• Dryness and peeling: Flaking of the treated skin.
• Increased photosensitivity: Greater susceptibility to sunburn.

Serious or Rare Side Effects

• Severe dermatitis: Widespread or intense skin inflammation.
• Allergic contact dermatitis: Hives, swelling or rash beyond the treated area.
• Ocular irritation: Eye redness or irritation if product contacts eyes.

When using Ketrel, several precautions should be observed to minimize adverse reactions and ensure safe handling.

• Application frequency: Follow prescriber instructions, typically once daily in the evening.
• Sun protection: Use broad‑spectrum sunscreen and protective clothing during daylight.
• Avoid concurrent irritants: Do not combine with other topical acne agents such as benzoyl peroxide unless directed.
• Pregnancy contraindication: Do not use during pregnancy (Category X) due to teratogenic risk.
• Discontinue if severe reaction: Stop treatment and seek medical advice if serious dermatitis develops.
• Store at room temperature: Keep below 25 °C in the original container, protected from moisture and light.
