Kiddir (Esomeprazole)

Esomeprazole belongs to the proton pump inhibitor class and is indicated for several acid‑related conditions.

• Gastro‑oesophageal reflux disease (GERD): alleviates heartburn and promotes healing of the esophageal lining.
• Erosive oesophagitis: heals erosive lesions and prevents further damage.
• NSAID‑induced gastric ulcer: protects the gastric mucosa and supports ulcer healing.
• Helicobacter pylori eradication (in combination therapy): enhances eradication success when used with antibiotics.

• Esomeprazole irreversibly binds to the H+/K+ ATPase enzyme (gastric proton pump) located on the secretory surface of parietal cells. By inhibiting this final step of acid production, it markedly reduces basal and stimulated gastric acid secretion, thereby increasing gastric pH and allowing mucosal healing in acid‑related disorders.

Adverse reactions may occur with esomeprazole therapy.

Common Side Effects

• Headache: mild to moderate pain often resolving without intervention.
• Diarrhea: loose stools that may be self‑limiting.
• Nausea: feeling of stomach upset, usually transient.
• Abdominal pain: discomfort in the upper abdomen.

Serious or Rare Side Effects

• Severe allergic reaction (anaphylaxis): requires immediate medical attention.
• Clostridioides difficile infection: may cause severe colitis.
• Low magnesium levels (hypomagnesemia): can lead to muscle cramps or cardiac arrhythmia.
• Liver enzyme elevation: indicates possible hepatic involvement.
• Interstitial nephritis: inflammation of kidney tissue.
• Stevens‑Johnson syndrome: rare but life‑threatening skin reaction.

Before initiating therapy, consider the following precautions.

• Assess renal function: adjust use in severe impairment.
• Monitor magnesium levels: long‑term treatment may cause hypomagnesemia.
• Review concomitant CYP2C19 inhibitors: avoid unless necessary.
• Use with caution in hepatic impairment: dose may need adjustment.

• Ketoconazole: markedly reduces its absorption; avoid co‑administration.
