• Telmisartan selectively antagonizes the angiotensin II type 1 (AT1) receptor, preventing angiotensin II‑mediated vasoconstriction, aldosterone secretion, and sympathetic activation. This blockade leads to vasodilation, reduced sodium and water retention, and ultimately a decrease in systemic vascular resistance and arterial pressure.

• Hepatic injury manifested by jaundice or elevated liver enzymes.

• Renal function: evaluate serum creatinine and eGFR, especially in patients with pre‑existing kidney disease.
