Klerimed (Clarithromycin)

Clarithromycin is indicated for bacterial infections affecting the respiratory tract, sinuses, skin and gastrointestinal system.

• Community‑acquired pneumonia: eradicates the causative bacteria and improves clinical symptoms.
• Acute bacterial sinusitis: reduces bacterial load and relieves sinus inflammation.
• Acute exacerbations of chronic bronchitis: shortens the duration of exacerbation and improves airflow.
• Skin and soft‑tissue infections: eliminates pathogenic bacteria and promotes wound healing.
• Helicobacter pylori eradication (in combination therapy): contributes to bacterial clearance as part of a multi‑drug regimen.

• Clarithromycin binds to the 50S subunit of bacterial ribosomes, blocking the translocation step of protein synthesis. This inhibition prevents elongation of the peptide chain, halting bacterial growth and leading to bacteriostatic or bactericidal effects depending on the organism and concentration.

Adverse reactions may range from mild to severe and should be monitored during therapy.

Common Side Effects

• Nausea: mild to moderate stomach upset.
• Diarrhea: frequent loose stools.
• Abdominal pain: cramping or discomfort.
• Taste disturbance: altered or metallic taste.
• Vomiting: occasional emesis.
• Headache: mild to moderate intensity.

Serious or Rare Side Effects

• Severe allergic reactions: rash, swelling, or anaphylaxis.
• QT interval prolongation: risk of cardiac arrhythmia.
• Clostridioides difficile infection: severe colitis.
• Hepatic dysfunction: elevated liver enzymes or hepatitis.
• Cardiac arrhythmias: ventricular tachycardia or fibrillation.
• Anaphylaxis: life‑threatening hypersensitivity.

Safe use of clarithromycin requires assessment of patient‑specific factors and proper handling.

• Assess renal and hepatic function: adjust dose if impairment is present.
• Review cardiac history: monitor QT interval in patients with arrhythmia risk.
• Avoid use in known macrolide allergy: discontinue if hypersensitivity occurs.
• Use with caution in pregnancy: limited data, weigh benefits against risks.
• Avoid concomitant use with other QT‑prolonging drugs: increase cardiac monitoring.
• Store at room temperature: keep in the original container, protect from moisture and light.

Clarithromycin can alter the plasma concentrations of several co‑administered medicines.

Major Interactions (Avoid)

• Erythromycin or other macrolides: additive toxicity and QT prolongation.
• Cisapride: heightened risk of serious arrhythmias.
• Astemizole: marked QT interval prolongation.
• Pimozide: severe cardiac effects, including torsades de pointes.

Moderate Interactions (Monitor Closely)

• Statins (simvastatin, lovastatin): increased risk of myopathy.
