Indications & Clinical Uses

Konakion Mixed Micelles (phytomenadione) is used to address disorders of vitamin K‑dependent coagulation.

  • Vitamin K deficiency bleeding: restores clotting factor activity to stop bleeding.
  • Newborn prophylaxis: prevents hemorrhagic disease of the newborn by supplying essential vitamin K.
  • Oral anticoagulant reversal: rapidly re‑establishes normal coagulation after warfarin or similar therapy.
  • Liver‑related coagulopathy: supports clotting factor synthesis when hepatic function is impaired.

How It Works

  • Phytomenadione is a synthetic form of vitamin K1 that acts as an essential cofactor for the enzyme gamma‑glutamyl carboxylase. This enzyme catalyzes the γ‑carboxylation of specific glutamic acid residues on clotting factors II, VII, IX, X and regulatory proteins C and S, enabling calcium binding and conversion to their biologically active forms, thereby restoring normal hemostasis. Adequate levels of phytomenadione are therefore required for the functional activation of the coagulation cascade.

Side Effects

Phytomenadione may be associated with several adverse reactions, ranging from mild to severe.

Common Side Effects

  • Injection site pain or erythema
  • Mild flushing or warmth sensation
  • Nausea or vomiting
  • Diarrhea
  • Transient elevation of liver enzymes
  • Headache

Serious or Rare Side Effects

  • Anaphylactic or severe hypersensitivity reactions
  • Hemolysis
  • Thrombosis or excessive clot formation
  • Severe bleeding due to over‑correction
  • Acute renal failure (rare)

Precautions & Warnings

Careful assessment and monitoring are recommended when using Konakion Mixed Micelles.

  • Pregnancy: use only if potential benefit justifies the potential risk to the fetus.
  • Breastfeeding: safety not established; consider risk versus benefit.
  • Renal impairment: monitor renal function and adjust therapy as needed.
  • Concomitant anticoagulants: may alter anticoagulant effect; monitor coagulation parameters closely.
  • Known hypersensitivity: avoid administration in patients with allergy to phytomenadione or formulation excipients.
  • Store at 2-8°C: keep refrigerated, protect from light, do not freeze or shake.

Avoid Interactions

Phytomenadione can interact with several medicines, influencing coagulation or vitamin K pathways.

Major Interactions (Avoid)

  • Concurrent use with high‑dose vitamin K antagonists may diminish reversal efficacy.
  • Combination with prothrombin complex
