Konakion (Phytomenadione)

Konakion is a vitamin K agent used to correct deficiencies and counteract anticoagulant effects.

• Vitamin K deficiency: restores functional clotting factors to prevent bleeding.
• Hemorrhagic disease of the newborn: provides prophylactic vitamin K to newborns at risk of bleeding.
• Anticoagulant reversal: rapidly normalizes coagulation in patients over‑anticoagulated by vitamin K antagonists.

• Phytomenadione acts as a cofactor for the gamma‑carboxylation of glutamate residues on clotting factors II, VII, IX and X. This enzymatic modification enables the factors to bind calcium ions, restoring their functional activity and allowing normal blood coagulation to resume.

Adverse reactions may occur with phytomenadione administration.

Common Side Effects

• Injection site pain or erythema
• Flushing or warmth sensation
• Headache
• Nausea or vomiting
• Diarrhoea
• Transient rise in liver enzymes

Serious or Rare Side Effects

• Anaphylactic or severe hypersensitivity reactions
• Acute hemolysis
• Severe hypotension
• Thrombosis in patients with underlying clotting disorders
• Persistent elevation of INR despite treatment

Use phytomenadione with clinical monitoring and appropriate patient assessment.

• Assess coagulation status: check baseline INR or PT before administration.
• Monitor INR/PT after dosing: ensure therapeutic correction without overshoot.
• Caution in liver disease: impaired synthesis of clotting factors may affect response.
• Avoid in known hypersensitivity to vitamin K or formulation excipients.
• Adjust dose in neonates: follow neonatal dosing guidelines.
• Store at ambient temperature: keep in original container, protect from moisture and light.

Phytomenadione may interact with several medications affecting coagulation or vitamin K metabolism.

Major Interactions (Avoid)

• Warfarin or other vitamin K antagonists: concurrent use may blunt anticoagulant effect.
• Broad‑spectrum antibiotics (e.g., cephalosporins): can reduce gut flora‑derived vitamin K, altering response.
• High‑dose vitamin K supplements: may cause excessive clotting.

Moderate Interactions (Monitor Closely)

• Oral contraceptives: may enhance clotting risk, monitor for thrombosis.
• Cholesterol‑lowering agents (e.g., statins): potential minor effect on vitamin K pathways.
• Antifungal agents (e.g., fluconazole): may alter vitamin K metabolism, require INR monitoring.
