Kytril (Granisetron)

Kytril (granisetron) is used to control nausea and vomiting mediated by serotonin receptors.

• Chemotherapy‑induced nausea and vomiting: prevents and reduces episodes in patients receiving emetogenic chemotherapy.
• Postoperative nausea and vomiting: reduces the incidence after surgical procedures.
• Radiation therapy‑induced nausea and vomiting: helps control symptoms in patients undergoing radiotherapy.

• Granisetron selectively blocks peripheral and central 5‑HT3 receptors, preventing serotonin from binding to these ligand‑gated ion channels. By inhibiting the serotonin‑mediated signaling pathway in the chemoreceptor trigger zone and gastrointestinal tract, it reduces the neural activation that triggers nausea and vomiting. This antagonism is rapid and sustained, providing effective antiemetic protection during and after emetogenic stimuli.

Granisetron may cause adverse reactions, most of which are mild and transient.

Common Side Effects

• Headache: mild to moderate headache.
• Constipation: reduced bowel movements.
• Dizziness: feeling of light‑headedness.
• Fatigue: general tiredness.
• Flushing: transient skin redness.

Serious or Rare Side Effects

• Cardiac arrhythmia: irregular heart rhythm, especially in patients with pre‑existing conditions.
• Severe allergic reaction: rash, swelling, difficulty breathing.
• Serotonin syndrome: rare but potentially life‑threatening when combined with serotonergic agents.
• Seizure: rare occurrence of convulsions.

Before administering Kytril, consider the following precautions.

• Assess cardiac history: monitor ECG in patients with known QT prolongation risk.
• Review concomitant serotonergic drugs: avoid or monitor for serotonin syndrome.
• Evaluate hepatic function: dose may need adjustment in severe impairment.
• Consider pregnancy status: use only if benefits outweigh risks.
• Monitor for allergic reactions: discontinue if severe hypersensitivity occurs.
• Store at 2-8°C: keep refrigerated, protect from light, do not freeze or shake.

Granisetron can interact with several drug classes; awareness helps prevent adverse events.

Major Interactions (Avoid)

• Other 5‑HT3 antagonists: additive antiemetic effect not required.
• Cisapride: increased risk of QT prolongation.
• Certain antiarrhythmics (e.g., quinidine, amiodarone): may exacerbate cardiac conduction abnormalities.

Moderate Interactions (Monitor Closely)

• Serotonergic agents (SSRIs, SNRIs, tramadol): risk of serotonin syndrome.
• CYP3A4 inhibitors (ketoconazole, erythromycin): may increase granisetron plasma levels.
• CYP3A4 inducers (rifampin, carbamazepine): may reduce efficacy.
