Lactulose Mip (Lactulose)

Lactulose is an osmotic laxative that acts on the gastrointestinal tract to modify stool consistency and reduce ammonia levels.

• Constipation: softens stool and promotes regular bowel movements.
• Hepatic encephalopathy: lowers blood ammonia by acidifying the colon and trapping ammonia for excretion.
• Prevention of ammonia accumulation in chronic liver disease: helps maintain lower systemic ammonia concentrations.

• After oral administration, lactulose reaches the colon where resident bacteria metabolize it to short‑chain fatty acids (lactic and acetic acid). This acidifies the colonic lumen, creating an osmotic gradient that retains water, softens stool, and accelerates transit. The acidic environment also converts ammonia (NH3) to non‑absorbable ammonium (NH4+), reducing systemic ammonia levels in hepatic encephalopathy.

Lactulose may cause a range of adverse reactions, most of which are mild and transient.

Common Side Effects

• Flatulence: increased intestinal gas production.
• Abdominal cramps: intermittent stomach discomfort.
• Diarrhea: loose stools, especially at higher doses.
• Nausea: feeling of queasiness.
• Bloating: sensation of fullness.
• Thirst: increased desire for fluids.

Serious or Rare Side Effects

• Severe electrolyte imbalance: especially low potassium.
• Dehydration: due to excessive fluid loss.
• Allergic reaction: rash, itching, or swelling.
• Severe abdominal pain: persistent or worsening pain.
• Rectal bleeding: unusual bleeding from the rectum.
• Hypotension: low blood pressure from fluid shifts.

Before prescribing lactulose, clinicians should consider several safety aspects.

• Renal function assessment: monitor kidney status, especially in patients with impairment.
• Electrolyte monitoring: check serum potassium and sodium during prolonged therapy.
• Diabetes caution: lactulose contains sugars; monitor blood glucose levels.
• Intestinal obstruction: avoid use if a mechanical blockage is present.
• Pregnancy and lactation: safety data are limited; use only if benefit outweighs risk.
• Store at room temperature: keep below 25 °C in the original container, protect from moisture.

Lactulose can interact with other medicines, altering efficacy or safety.
