Lactulose is used in gastrointestinal and hepatic conditions where reduction of intestinal ammonia or promotion of bowel movements is required.

• Constipation: softens stool and promotes regular bowel movements.
• Hepatic encephalopathy: lowers colonic ammonia production to improve neurological symptoms.
• Prevention of hepatic encephalopathy recurrence: maintains low ammonia levels to reduce episodes in chronic liver disease.

• After oral administration, lactulose reaches the colon unchanged, where bacterial enzymes hydrolyze it to short‑chain fatty acids such as lactic and acetic acid. The resulting acidification lowers colonic pH and creates an osmotic gradient that draws water into the lumen, softening stool. The acidic environment also traps ammonia as ammonium ions, decreasing its absorption and thereby lowering systemic ammonia levels.

Lactulose may cause a range of gastrointestinal and systemic reactions, most of which are mild and dose‑related.
