Lactulose is an osmotic laxative used to manage conditions related to bowel motility and ammonia accumulation.

• Constipation: softens stool and promotes regular bowel movements.

• Prevention of hepatic encephalopathy recurrence: maintains low ammonia concentrations after an acute episode.

• Lactulose is a synthetic disaccharide that reaches the colon unchanged, where bacterial β‑galactosidase hydrolyzes it to lactic acid, acetic acid and other short‑chain fatty acids. The resulting acidic environment traps ammonia as ammonium ions and creates an osmotic gradient that draws water into the lumen, softening stool and accelerating transit.

Lactulose may cause a range of adverse reactions, most of which are mild and transient.

Common Side Effects

• Abdominal cramps: discomfort due to increased intestinal activity.
• Flatulence: excess gas production from bacterial fermentation.
• Diarrhea: loose stools from osmotic water retention.

• Electrolyte monitoring: check potassium and sodium levels during prolonged therapy.
• Hepatic impairment: use with caution in advanced liver disease.
• Galactosemia avoidance: contraindicated in patients with this metabolic disorder.
