• Lactulose remains largely unabsorbed in the small intestine, increasing osmotic pressure in the lumen and retaining water, which softens stool and stimulates peristalsis. In the colon, bacterial metabolism converts lactulose to lactic and acetic acids, lowering colonic pH and converting ammonia to non‑absorbable ammonium, thereby reducing systemic ammonia levels.
