Lannapril Plus (Hydrochlorothiazide, Ramipril)

Lannapril Plus, a combination ACE inhibitor and thiazide diuretic, is used in the management of hypertension.

• Hypertension: lowers systolic and diastolic blood pressure and reduces cardiovascular risk.
• Essential hypertension: provides sustained blood pressure control when monotherapy is insufficient.
• High blood pressure in adults: improves overall vascular health and decreases the likelihood of stroke or myocardial infarction.

• Ramipril inhibits angiotensin‑converting enzyme, decreasing formation of angiotensin II and leading to vasodilation and reduced aldosterone secretion. Hydrochlorothiazide blocks Na⁺/Cl⁻ reabsorption in the distal convoluted tubule, increasing urinary sodium and water excretion, which lowers plasma volume. The combined actions result in decreased peripheral resistance and blood volume, producing an additive antihypertensive effect.

Common Side Effects

• Cough: dry, persistent throat irritation.
• Dizziness or light‑headedness, especially after the first dose.

• Fatigue: general feeling of tiredness.

Serious or Rare Side Effects

• Angioedema: swelling of face, lips, tongue, or airway.

• Acute kidney injury: sudden reduction in renal function.
• Hyperkalemia: elevated potassium levels, potentially cardiac.
• Blood dyscrasias: such as leukopenia or thrombocytopenia.
• Severe dehydration: excessive fluid loss leading to electrolyte imbalance.

• Blood pressure monitoring: ensure adequate control and avoid excessive hypotension.
• Renal function assessment: check serum creatinine and eGFR prior to and during treatment.
• Electrolyte surveillance: monitor potassium and sodium levels regularly.
