Lantan (Lanthanum)

Lantan (lanthanum) is employed as a phosphate binder to manage elevated serum phosphate in chronic kidney disease.

• Hyperphosphatemia in chronic kidney disease: reduces intestinal phosphate absorption to lower serum phosphate levels.
• Patients on dialysis: helps achieve target phosphate concentrations recommended by nephrology guidelines.

• Lanthanum carbonate dissociates in the acidic environment of the stomach, releasing lanthanum ions that bind dietary phosphate to form insoluble lanthanum phosphate complexes. These complexes are not absorbed and are eliminated in the feces, thereby decreasing the amount of phosphate entering the systemic circulation and helping to control serum phosphate concentrations in chronic kidney disease patients.

Lantan may cause a range of adverse reactions, most of which are mild and reversible.

Common Side Effects

• Nausea: mild to moderate discomfort in the stomach.
• Constipation: reduced bowel movements, sometimes requiring dietary adjustment.

• Allergic reactions: rash, itching, or swelling that may indicate hypersensitivity.
• Liver enzyme elevation: abnormal liver function tests observed in rare cases.

When prescribing Lantan, clinicians should consider several safety measures.

• Monitor serum phosphate: regular blood tests to ensure levels remain within target range.
• Assess calcium status: avoid concurrent high calcium intake that could lead to hypercalcemia.
• Evaluate bowel health: contraindicated in patients with known bowel obstruction or severe constipation.
• Adjust for other binders: coordinate dosing when other phosphate binders are used to prevent over‑binding.
• Review concomitant medications: antacids or iron supplements may reduce efficacy and should be spaced appropriately.
