Indications & Clinical Uses

Clopidogrel is indicated for reducing platelet aggregation in cardiovascular disease states.

  • Atherosclerotic disease: lowers risk of myocardial infarction, stroke and cardiovascular death.
  • Acute coronary syndrome: prevents recurrent ischemic events such as heart attack.
  • Post‑percutaneous coronary intervention (PCI): reduces incidence of stent thrombosis and related complications.

How It Works

  • Clopidogrel is a pro‑drug that is metabolized by hepatic cytochrome P450 enzymes to an active thiol metabolite. This metabolite binds irreversibly to the platelet P2Y12 ADP receptor, blocking ADP‑mediated activation of the glycoprotein IIb/IIIa complex and thereby inhibiting platelet aggregation and thrombus formation. The irreversible binding persists for the lifespan of the platelet, approximately 7‑10 days, providing sustained antiplatelet effect until new platelets are generated.

Clopidogrel may cause a variety of adverse reactions, ranging from common, mild events to rare but serious conditions.
