Lasix (Furosemide)

Lasix (furosemide) acts on the renal loop of Henle to reduce fluid retention in several clinical conditions.

• Congestive heart failure edema: reduces excess fluid accumulation and improves breathing and peripheral swelling.
• Pulmonary edema: promotes rapid removal of fluid from the lungs, aiding oxygenation.
• Renal impairment with fluid overload: facilitates diuresis when kidney function is compromised.
• Hypertension: lowers blood volume, contributing to blood pressure reduction.

• Furosemide inhibits the Na⁺/K⁺/2Cl⁻ cotransporter located in the thick ascending limb of the loop of Henle. By blocking this transporter, it decreases reabsorption of sodium, potassium, and chloride, leading to increased excretion of these ions together with water (natriuresis and diuresis). The resulting reduction in extracellular fluid volume lowers venous return and arterial pressure, producing its therapeutic diuretic effect.

Furosemide may cause a range of adverse reactions, varying from mild to severe.

Common Side Effects

• Increased urination: frequent need to void.
• Electrolyte imbalance: low potassium or sodium levels.
• Dehydration: reduced body water leading to dizziness.
• Hypotension: drop in blood pressure causing light‑headedness.
• Dizziness or headache.
• Tinnitus: ringing in the ears.

Serious or Rare Side Effects

• Ototoxicity: hearing loss or persistent ringing.
• Severe electrolyte disturbances: hyperkalemia or severe hyponatremia.
• Acute renal failure: sudden decline in kidney function.
• Allergic reactions: rash, itching, or anaphylaxis.
• Thrombocytopenia: low platelet count.
• Pancreatitis: inflammation of the pancreas.

When using Lasix, careful monitoring and specific precautions help minimize risks.

• Monitor electrolytes: check serum potassium, sodium and magnesium regularly.
• Assess renal function: evaluate creatinine and urine output before and during therapy.
• Use lowest effective dose: titrate to achieve desired diuresis while limiting side effects.
• Caution in hypotension: avoid rapid administration in patients with low blood pressure.
• Avoid in anuria: do not use when urine output is absent.
• Store at room temperature: keep in original packaging, protect from moisture and light.

• Aminoglycoside antibiotics: combined use may increase risk of ototoxicity.
• Cisplatin: additive nephrotoxic effects.
