Lasix (Furosemide)

Furosemide is a loop diuretic that acts on the renal tubular system to reduce fluid overload.

• Edema associated with heart failure: reduces excess fluid and improves breathing and swelling.
• Edema associated with liver cirrhosis: decreases abdominal and peripheral fluid accumulation.
• Edema associated with renal disease: facilitates fluid removal in nephrotic syndrome.
• Hypertension: lowers blood pressure by promoting diuresis.

• Furosemide inhibits the Na⁺‑K⁺‑2Cl⁻ cotransporter in the thick ascending limb of the loop of Henle, leading to reduced reabsorption of sodium, chloride, and potassium. This disruption increases urinary excretion of these electrolytes together with water, producing a potent diuretic effect that lowers extracellular fluid volume and blood pressure.

Furosemide may cause a range of adverse reactions, from mild to severe.

Common Side Effects

• Increased urination: frequent need to void.
• Electrolyte imbalance: low potassium or sodium levels.
• Dizziness or light‑headedness, especially on standing.
• Headache: mild to moderate intensity.
• Blurred vision: temporary visual disturbances.
• Dehydration: feeling thirsty or dry mouth.

Serious or Rare Side Effects

• Severe dehydration: can lead to hypotension and organ dysfunction.
• Ototoxicity: hearing loss or ringing in the ears, especially with high doses.
• Allergic reactions: rash, itching, or swelling.
• Acute kidney injury: sudden decline in renal function.
• Extreme electrolyte disturbances: dangerous low potassium or sodium.
• Severe hypotension: marked drop in blood pressure causing fainting.

Before initiating Lasix, several safety considerations should be reviewed.

• Monitor electrolytes: regular blood tests for potassium, sodium, and magnesium.
• Assess renal function: check creatinine and eGFR prior to and during therapy.
• Adjust dose in hepatic impairment: reduced liver function may increase drug exposure.
• Use caution in diabetes: diuresis can affect blood glucose control.
• Avoid in anuria: patients unable to produce urine should not receive loop diuretics.
• Store at room temperature: keep in the original pack, protect from moisture and direct sunlight.

Furosemide can interact with other medicines, altering its effectiveness or safety.

Major Interactions (Avoid)

• NSAIDs: may blunt the diuretic response and retain fluid.
• Lithium: increased lithium concentrations, raising toxicity risk.
• Aminoglycoside antibiotics: heightened risk of ototoxicity when combined.

Moderate Interactions (Monitor Closely)

• ACE inhibitors or ARBs: additive blood‑pressure lowering effect.
• Digoxin: electrolyte changes can predispose to digoxin toxicity.
• Other antihypertensive agents: may cause excessive hypotension.
