Lasix (Furosemide Sodium)

Furosemide is a loop diuretic that acts on the renal tubular system to promote fluid removal.

• Edema associated with congestive heart failure: reduces fluid accumulation and improves symptoms.
• Edema due to hepatic cirrhosis: facilitates removal of excess ascitic fluid.
• Edema secondary to renal disease: helps manage fluid overload in nephrotic conditions.
• Hypertension: lowers blood pressure by decreasing plasma volume.

• Furosemide inhibits the Na⁺/K⁺/2Cl⁻ cotransporter in the thick ascending limb of the loop of Henle, reducing reabsorption of sodium, chloride, and potassium. This disruption creates an osmotic gradient that draws water into the tubular lumen, increasing urinary output and decreasing extracellular fluid volume, which underlies its diuretic and antihypertensive effects. The resultant natriuresis and diuresis also lead to decreased preload and afterload, contributing to blood pressure reduction.

Furosemide may cause a range of adverse reactions, varying from mild to severe.

Common Side Effects

• Increased urination: frequent or large‑volume urine output.
• Electrolyte imbalance: low potassium, sodium, or magnesium levels.
• Dizziness or lightheadedness: due to volume depletion.
• Headache.
• Muscle cramps.
• Dehydration: signs of thirst, dry mouth.

Serious or Rare Side Effects

• Severe electrolyte disturbances: hypokalemia or hyponatremia leading to cardiac arrhythmias.
• Ototoxicity: hearing loss or ringing in the ears, especially with rapid IV administration.
• Acute kidney injury: sudden rise in serum creatinine.
• Severe hypotension: marked drop in blood pressure causing fainting.
• Allergic reactions: rash, itching, swelling, or anaphylaxis.

When using Lasix, careful monitoring and patient assessment are essential.

• Renal function monitoring: assess serum creatinine and electrolytes before and during therapy.
• Electrolyte surveillance: check potassium, sodium, magnesium regularly.
• Volume status assessment: monitor blood pressure and signs of dehydration.
• Concomitant ototoxic drugs: avoid or use with caution when combined with aminoglycosides or other loop diuretics.
• Pregnancy and lactation: use only if clearly needed, as safety data are limited.
