This results in decreased leukocyte migration, reduced edema, and inhibition of keratinocyte proliferation, providing anti‑inflammatory and immunosuppressive effects in the skin. Additionally, it down‑regulates phospholipase A2 activity, limiting arachidonic acid release and downstream prostaglandin synthesis.

Topical application of hydrocortisone butyrate may be associated with local and, rarely, systemic adverse reactions.
