Lattulac (Lactulose)

Lattulac is an osmotic laxative indicated for conditions affecting bowel function and hepatic metabolism.

• Constipation: softens stool and promotes regular bowel movements by increasing intestinal water content.
• Hepatic encephalopathy: lowers blood ammonia levels, helping improve neurological symptoms in patients with liver disease.

• Lactulose is a synthetic disaccharide that is not absorbed in the small intestine. In the colon, bacterial enzymes hydrolyze it to short‑chain acids, lowering colonic pH and creating an osmotic gradient that draws water into the lumen, facilitating stool passage. The acidic environment also converts ammonia to ammonium, reducing its absorption and systemic levels in hepatic encephalopathy.

Lactulose may cause a range of adverse reactions, most of which are mild and transient.

Common Side Effects

• Bloating: sensation of abdominal fullness.
• Flatulence: increased intestinal gas.
• Diarrhea: loose stools, especially at higher doses.
• Abdominal cramps: intermittent stomach pain.
• Nausea: feeling of sickness.
• Vomiting: expulsion of stomach contents.

Serious or Rare Side Effects

• Severe electrolyte imbalance: especially low potassium.
• Dehydration: excessive fluid loss from diarrhea.
• Allergic reaction: rash, itching, swelling, or difficulty breathing.
• Hyperosmolar coma: extremely rare, due to profound osmotic shifts.

When prescribing lactulose, several safety considerations should be observed.

• Dose titration: adjust based on stool frequency and patient tolerance.
• Fluid intake: maintain adequate hydration to prevent dehydration.
• Renal impairment: use with caution, monitor renal function.
• Diabetes: monitor blood glucose as lactulose may affect sugar levels.
• Electrolyte monitoring: watch for potassium loss during prolonged use.
• Store at room temperature: keep in the original container, protect from moisture and heat.

Lactulose can interact with other medicines, requiring attention to avoid reduced efficacy or increased toxicity.
