Lactulose acts as an osmotic laxative and ammonia‑lowering agent in the gastrointestinal tract.

• Prevention of hepatic encephalopathy: reduces intestinal ammonia production, helping to prevent neurocognitive deterioration in patients with chronic liver disease.
• Treatment of hepatic encephalopathy: lowers systemic ammonia levels, improving mental status and reducing episodes of hepatic encephalopathy.

• After oral ingestion, lactulose reaches the colon where bacterial enzymes hydrolyze it to short‑chain fatty acids, primarily lactic and acetic acid. This acidification lowers colonic pH, converting ammonia (NH3) to ammonium (NH4+), which is trapped in the lumen, drawing water into the bowel and promoting laxation while simultaneously decreasing systemic ammonia absorption.
