Lattulosio Sandoz (Lactulose)

Lattulosio Sandoz is an osmotic laxative used in gastrointestinal and hepatic conditions.

• Constipation: softens stools and promotes regular bowel movements.
• Hepatic encephalopathy: lowers colonic ammonia absorption, helping improve neurological symptoms.

• Lactulose is a synthetic disaccharide that is not absorbed in the small intestine. In the colon it is metabolized by bacteria into acidic compounds, increasing osmotic pressure and drawing water into the lumen, which softens stool and stimulates peristalsis. The resulting acidification also converts ammonia to ammonium, reducing its absorption and systemic levels.

Lactulose may cause a range of adverse reactions, most of which are mild and transient.

Common Side Effects

• Abdominal bloating: feeling of fullness or swelling in the abdomen.
• Flatulence: increased intestinal gas.
• Diarrhea: loose, watery stools.
• Nausea: sensation of unease or urge to vomit.
• Stomach cramps: intermittent abdominal pain.
• Thirst: increased desire for fluids.

Serious or Rare Side Effects

• Severe electrolyte imbalance: abnormal sodium or potassium levels.
• Dehydration: excessive fluid loss leading to low blood volume.
• Allergic reaction: rash, itching, swelling, or difficulty breathing.
• Worsening hepatic encephalopathy: rare paradoxical increase in ammonia.
• Severe abdominal pain: persistent or intense discomfort.
• Hypoglycemia in diabetic patients: low blood sugar due to altered carbohydrate metabolism.

Before initiating therapy, consider the following safety measures.

• Renal function assessment: adjust use in patients with impaired kidney function.
• Fluid and electrolyte monitoring: watch for signs of dehydration or electrolyte disturbances.
• Hepatic encephalopathy titration: dose should be individualized to achieve desired clinical response.
• Avoid abrupt discontinuation: sudden stop may lead to rebound constipation.
• Store at ambient temperature below 25°C: keep in the original container, protect from moisture.

Lactulose can interact with several medicines, requiring attention to avoid reduced efficacy or increased adverse effects.

• Antibiotics (e.g., rifaximin): may diminish lactulose’s effect on ammonia reduction.
• Other laxatives: additive osmotic action can cause severe diarrhea.
• Antidiarrheal agents (e.g., loperamide): antagonize lactulose’s intended laxative action.
