Ledronin (Alendronate Sodium Trihydrate)

Ledronin (alendronate) acts on the bone remodeling pathway to reduce excessive bone loss. It is indicated for the following conditions:

• Postmenopausal osteoporosis: reduces the risk of vertebral and non‑vertebral fractures by inhibiting osteoclast‑mediated bone resorption.
• Glucocorticoid‑induced osteoporosis: helps maintain bone density in patients receiving long‑term glucocorticoid therapy.
• Paget's disease of bone: normalises abnormal bone turnover and alleviates associated pain.

• Alendronate Sodium Trihydrate binds avidly to hydroxyapatite crystals on bone surfaces, especially at sites of active remodeling. When osteoclasts attach to the bone, the drug is internalised and inhibits farnesyl pyrophosphate synthase in the mevalonate pathway, preventing prenylation of essential proteins. This disruption reduces osteoclast activity and induces apoptosis, leading to decreased bone resorption and a net gain in bone mineral density.

Adverse reactions may occur with oral bisphosphonates. Patients should be aware of both common and serious possibilities.

Common Side Effects

• Gastrointestinal irritation: dyspepsia, heartburn, or esophageal discomfort.
• Nausea or vomiting.
• Abdominal pain or cramping.
• Musculoskeletal pain: aches in muscles or joints.
• Headache.
• Transient flu‑like symptoms.

Serious or Rare Side Effects

• Esophageal ulceration or severe esophagitis.
• Atypical femur fracture.
• Severe musculoskeletal pain persisting for weeks.
• Hypocalcemia.
• Osteonecrosis of the jaw.
• Acute renal impairment.

Before initiating therapy, several safety considerations are recommended:

• Renal function: assess creatinine clearance and avoid use in severe renal impairment.
• Calcium/Vitamin D status: ensure adequate supplementation to minimise hypocalcemia.
• Swallow whole: do not chew, split, or crush tablets to prevent esophageal injury.
• Timing and posture: take with plain water on an empty stomach and remain upright for at least 30 minutes.
• Contraindications: avoid in patients with known hypersensitivity to alendronate or other bisphosphonates.
• Store at ambient temperature: keep in the original container, protect from moisture and heat.

Alendronate may interact with several medicines, requiring attention to avoid reduced efficacy or increased toxicity.

Major Interactions (Avoid)

• Calcium‑containing antacids or supplements: markedly decrease alendronate absorption.
• Iron supplements: impair oral bioavailability of alendronate.
• Magnesium‑containing laxatives: reduce drug absorption.

Moderate Interactions (Monitor Closely)

• Systemic corticosteroids: may heighten risk of hypocalcemia.
• Thiazide diuretics: can affect calcium balance and increase serum calcium.
• Warfarin: potential alteration of INR, requiring closer coagulation monitoring.
