Lenalidomide is employed in hematologic oncology to target malignant plasma cells and abnormal myeloid clones.

• Lenalidomide binds to the cereblon component of the CRL4 E3 ubiquitin ligase complex, altering substrate specificity and promoting degradation of transcription factors Ikaros and Aiolos. This results in modulation of cytokine production, inhibition of angiogenesis, enhancement of natural killer and T‑cell mediated immunity, and induction of apoptosis in malignant plasma cells and del(5q) myelodysplastic clones.
