Lenalidomide is used in hematologic malignancies and related myelodysplastic conditions.

• Lenalidomide binds to the cereblon subunit of the CRL4‑E3 ubiquitin ligase complex, altering substrate specificity. This leads to ubiquitination and proteasomal degradation of transcription factors such as Ikaros and Aiolos, resulting in anti‑angiogenic, immunomodulatory, and anti‑proliferative effects on malignant plasma cells and other hematologic tumor cells. The modulation of cytokine production and enhancement of natural killer cell activity further contribute to its therapeutic activity.
