Lenalidomide is an immunomodulatory agent indicated for specific hematologic malignancies and related cytogenetic disorders.

• Multiple myeloma: improves progression‑free survival, overall response rates, and depth of response in patients with relapsed or refractory disease, often in combination with dexamethasone.
• Myelodysplastic syndromes (5q deletion): reduces transfusion dependence, induces cytogenetic remission, and prolongs survival in adult patients with the del(5q) chromosomal abnormality.

• Lenalidomide binds to the cereblon component of the CRL4‑E3 ubiquitin ligase complex, altering substrate specificity and leading to degradation of transcription factors Ikaros and Aiolos. This results in inhibition of angiogenesis, modulation of cytokine production, and enhanced activation of T‑cells and natural killer cells, collectively producing anti‑tumor activity in malignant plasma cells and dysplastic hematopoietic clones. The drug also reduces adhesion of myeloma cells to the bone marrow microenvironment, further impairing tumor growth.
