Lenalidomide Stada (Lenalidomide)

• Lenalidomide is an immunomodulatory antineoplastic used in several hematologic malignancies.
• Multiple Myeloma: improves disease control and prolongs survival in relapsed or refractory patients.
• Myelodysplastic Syndromes with del(5q): reduces transfusion dependence and can induce cytogenetic remission.
• Mantle Cell Lymphoma: provides therapeutic benefit in relapsed or refractory disease.

• Lenalidomide binds to the cereblon component of the CRL4‑E3 ubiquitin ligase complex, promoting ubiquitination and proteasomal degradation of transcription factors Ikaros and Aiolos. This leads to anti‑proliferative, pro‑apoptotic and anti‑angiogenic effects while modulating T‑cell and NK‑cell activity.

Lenalidomide may cause a range of adverse reactions.

Common Side Effects

• Fatigue: persistent tiredness reported by many patients.
• Nausea: mild to moderate upset stomach.
• Diarrhea: loose stools, often self‑limiting.
• Rash: skin irritation or erythema.
• Cytopenias: reduced neutrophils, platelets or red cells.
• Peripheral neuropathy: tingling or numbness in extremities.

Serious or Rare Side Effects

• Deep vein thrombosis or pulmonary embolism.
• Secondary primary malignancies.
• Severe neutropenia or febrile neutropenia.
• Hepatotoxicity: elevated liver enzymes or liver injury.

• Before initiating therapy, several safety considerations are required.
• Pregnancy contraindication: drug is classified as Category X; avoid use in pregnant women.
• Thromboembolic risk: assess need for prophylactic anticoagulation.
• Hematologic monitoring: regular CBCs to detect cytopenias.
• Renal impairment: dose adjustment may be necessary in reduced kidney function.
• Drug interaction vigilance: review concomitant medications for CYP3A4 modulators.
• Store at room temperature: keep in original packaging, protect from moisture and light.

Major Interactions (Avoid)

• Strong CYP3A4 inducers (e.g., rifampin, carbamazepine) – may lower lenalidomide exposure.
• P‑gp inducers – can reduce absorption and efficacy.

Moderate Interactions (Monitor Closely)

• CYP3A4 inhibitors (e.g., ketoconazole) – may raise plasma levels.
