Leponex (Clozapine)

Leponex (clozapine) is used in the management of severe psychotic disorders that are resistant to standard therapy.

• Treatment‑resistant schizophrenia: provides symptom control and reduces relapse risk in patients who have not responded adequately to at least two other antipsychotics.
• Schizoaffective disorder: improves both mood and psychotic symptoms in individuals whose condition is not adequately managed by other treatments.

• Clozapine acts as an antagonist at dopamine D2 receptors and serotonin 5‑HT2A receptors, while also binding to dopamine D1, D4, α‑adrenergic, histamine H1 and muscarinic receptors. This combined modulation of dopaminergic and serotonergic pathways reduces positive psychotic symptoms and offers a lower risk of extrapyramidal side effects compared with typical antipsychotics.

Clozapine may cause a range of adverse reactions, varying from mild to potentially life‑threatening.

Common Side Effects

• Sedation: feeling of drowsiness or fatigue.
• Weight gain: increase in body weight and appetite.
• Constipation: reduced bowel movements leading to discomfort.
• Salivation: excessive drooling.

Serious or Rare Side Effects

• Agranulocytosis: severe reduction in white blood cells requiring regular monitoring.
• Myocarditis: inflammation of the heart muscle that can be fatal.
• Seizures: risk of convulsive episodes, especially at higher doses.

When prescribing clozapine, clinicians must observe several safety measures to minimize risks.

• Blood monitoring: weekly complete blood count for the first 6 months, then bi‑weekly or monthly as advised.
• Cardiovascular assessment: baseline ECG and periodic checks for myocarditis or cardiomyopathy.
• Metabolic monitoring: track weight, glucose and lipid levels due to risk of metabolic syndrome.
• Concomitant medications: avoid drugs that lower seizure threshold or cause significant drug interactions.
• Pregnancy: use only if benefits outweigh risks; discuss with obstetric specialist.
• Store at ambient temperature: keep tablets in original container, protect from moisture and light.

Clozapine interacts with several medicines; some combinations must be avoided while others require careful monitoring.

Major Interactions (Avoid)

• CYP1A2 inhibitors (e.g., fluvoxamine, ciprofloxacin): can markedly increase clozapine plasma levels.
• Smoking cessation: loss of CYP1A2 induction may raise drug concentrations.
• Concomitant use with other antipsychotics: increases risk of QT prolongation and neuroleptic malignant syndrome.

Moderate Interactions (Monitor Closely)

• CYP3A4 inducers (e.g., carbamazepine, rifampicin): may reduce clozapine effectiveness.
• CNS depressants (e.g., benzodiazepines, alcohol): enhance sedation and respiratory depression.
• Anticholinergic agents: may exacerbate constipation and urinary retention.
